Eligibility Conditions

The applicant must not have a family name recorded on their birth registration.

Required Documents

  • A request addressed to the Minister of the Interior, authenticated by the applicant’s signature, stating the full address.
  • The request must be accompanied by the applicant’s birth extract.
  • Birth or death extract (of the applicant’s father).
  • Birth extracts of the applicant’s siblings showing the different family name they carry.
  • A copy of the applicant’s ID card.

Service Steps

  1. Submit a request to correct birth records, authenticated by the holders’ signatures, to the civil status officer (jurisdiction based on place of birth).
  2. The request must be supported by the required documents.
  3. The Prime Ministry is notified of the new family name so it can be published in the Official Gazette of the Tunisian Republic.
  4. The municipality is notified of the new family name to record it in its registers and correct the applicant’s family name.
